A Medical Coding (HCC) department is responsible for accurately assigning standardized codes to patient diagnoses and procedures, primarily using the Hierarchical Condition Categories (HCC) system, to support risk adjustment and proper reimbursement. These coders review medical records, translate clinical information into codes, and ensure compliance with coding guidelines and regulations.
Key Responsibilities of an HCC Coder:
1>>Review and Analyze Medical Records:
Examine patient charts, documentation, and medical records to identify relevant diagnoses and procedures for coding.
2>>Assign Codes:
Translate clinical information into appropriate ICD-10-CM codes, ensuring correct mapping to HCCs for risk adjustment.
3>>Maintain Coding Accuracy and Compliance:
Ensure all coding is accurate, consistent, and compliant with industry regulations, including HIPAA and coding guidelines.
4>>Collaborate with Healthcare Providers:
Work with physicians and other healthcare professionals to clarify documentation and resolve coding discrepancies.
