An E/M (IP OP) medical coder’s job is to accurately review and analyze patient medical records to assign appropriate codes (ICD-10, CPT, HCPCS) for Evaluation and Management (E&M) services, as well as both Inpatient (IP) and Outpatient (OP) services, ensuring compliance with coding guidelines and regulatory requirements. This role involves working closely with physicians and billing teams to resolve documentation discrepancies, conducting quality checks, and maintaining coding accuracy standards.
Responsibilities:
Review and analyze clinical documentation: Identify the relevant information in patient records to determine the appropriate medical codes.
Assign applicable codes: Utilize E&M codes for both IP and OP services, along with other relevant codes (ICD-10, CPT, HCPCS).
Ensure coding accuracy and compliance: Adhere to all regulatory guidelines and billing requirements, including CMS (Centers for Medicare & Medicaid Services) regulations.
Maintain current coding resources: Keep coding books and guidelines updated with the latest information.
Communicate effectively with healthcare providers: Collaborate with physicians to clarify documentation and ensure accuracy.
Participate in coding audits: Contribute to quality improvement by participating in coding audits and implementing feedback.
Assist with resolving coding issues: Help address any coding-related problems that may impact the billing process.
